Wellbeing Group Referrals FAQ/Fact Sheets

Q1. I’m not a medical doctor, but I am a registered health professional. Can I make a referral to Hearts & Minds Wellbeing Support Groups?
A1. Yes, we accept referrals from a range of different professionals who work in healthcare, including registered nurses, counsellors, psychologists, social workers, and occupational therapists.
If you are still unsure if your profession is able to refer, please call us on 09 441 8989 to discuss this.
Q2. How can I make a referral to Hearts & Minds Wellbeing Support Groups?
A2. You can make a referral to Hearts & Minds Wellbeing Support Groups by filling out our referral form (this can be downloaded from our website), and sending it back to us through one of the following methods:

  • Or you can us our online referral form at www.heartsandminds.org.nz
Q3. What are the criteria for referral?
A4. The criteria for clients to be able to access our WDHB funded Wellbeing Support Groups are, that clients:
  • Have given their consent for the referral.
  • Are at least 18 years old.
  • Live, or access health services, within the Waitemata District Health Board catchment area.
  • Be experiencing mild to moderate mental health concerns only.
Q4. What does ‘mild to moderate’ mental health concerns mean?
A4. Mild to moderate refers to clients whose wellbeing is affected by their mental health, however they are still stable in themselves and able to act independently in their everyday lives, and are not at risk of harming themselves or others.
Q5. How do I add a tick to the box on the Word document referral form?
A5. If you don’t want to print the form out, you should be able to right click the checkbox and select “properties” then just change the default value to “checked” for that referral. If this doesn’t work, please bold the text that you would like to select.
Q6. Do I have to use the Hearts & Minds referral form, or can I use the same referral process as we use for other services?
A6. The Hearts & Minds referral form is our preferred format for receiving referrals, however we accept referrals in a variety of formats. The only requirements are that a referral includes all the information that we ask for on our referral form, and meets our criteria for referral.
The key information that we require:
  • The client’s full name
  • The client’s age or date of birth (at this time, we can only accept referrals for clients who are 18 years old or over.)
  • The client’s contact details (at minimum we need a current daytime contact phone number)
  • The reason for the referral (e.g. anxiety, depression, self-esteem/confidence, etc.)
  • Whether the client has given their consent for the referral
  • Any risk factors for the client (e.g. suicidal ideation, self-harm, psychosis, violent behaviour)
Q7. Can I register my client/patient for a Wellbeing Support Group?
A7. No, this is something we need to do directly with the client. Once we have received your referral we will contact your client in order to go through the registration process with them.
